Class: Mucolytic.
Pharmacology
Carbocisteine reduces the viscosity of bronchial secretions and facilitates expectoration. It alters the physical and chemical characteristics of the mucin components of sputum to a more ‘normal’ pattern (by reducing fructose and sulphate content and increasing the proportion of sialomucins). In patients with COPD, mucolytics reduce the number of exacerbations and days of illness but the benefit appears small and their routine use remains debatable.1,2 However, some individual patients benefit from their use and a therapeutic trial may be justified if all other approaches have failed, particularly in patients with more severe COPD and frequent or prolonged exacerbations.
